Workshop on sharing experiences in implementing the Transition house for rehabilitation

In the morning of April 9, 2021, within the framework of the project "Strengthening opportunities and enhancing the status of persons with disabilities" funded by the U.S. Agency for International Development in Vietnam, ACDC organized a Workshop on Sharing experiences in implementing the Transition House for rehabilitation in Quang Tri.

The objective of the workshop was to share experiences in establishing and operating the Transition House model at medical facilities on routes; assess the effectiveness of the Transition Home model and propose directions for maintaining, developing, and replicate the model in medical facilities; at the same time, share solutions to improve the efficiency of the transition home model after persons with disabilities return to the community.

The workshop was attended by Ms. Ritu K Tariyal, Director of The Department of Environment and Social Development, USAID; Mr. Aaron Schulson Brownell, Deputy Director of Program Development Department; PGS-TS. Tran Trong Hai – Chairman of Vietnam Association of Rehabilitation; Ms. Le Tuan Dong – Advisor of Community Development Research Institute; 3. Mr. Nguyen Thi Lan Anh – Director of ACDC; representatives from the Ministry of Health; Department of Health, Department of Labor, War Invalids and Social Affairs of 3 provinces Thua Thien – Hue, Quang Tri, Quang Nam; leaders and staff of hospitals, medical facilities under 3 provinces ...

Aaron Schulson Brownell, Deputy Director of USAID's Program Development Division, said at the workshop: "I congratulate you on your outstanding efforts, commitments and efforts to bring the transition house model to the people. We are at the dawn of this model, I congratulate you and wish you continued success."

At the seminar, participants listened to discussions on the construction process, initial results, experiences acquired from medical facilities with a transition home model. At the same time, the workshop was also an opportunity for delegates to discuss difficulties and challenges when implementing the model as well as the impact and ability to replicate at health facilities at all levels. The delegates agreed that this is a useful model, capable of replicated and highly effective for patients and persons with disabilities in rehabilitation and community inclusion.

The Transition Home model is established to support improving the quality of life of patients and persons with disabilities after rehabilitation. The Transport House model is a combination of 2 fields and physical access and healthcare. Before returning home from rehabilitation treatment, patients and persons with disabilities are encouraged to use this model as a good environment to re-practice lost or impaired living functions caused by illness; and references to outreach designs or other related issues, helping persons with disabilities include better into life.

This model has been built and tested at 04 medical facilities including Quang Tri Hospital of Traditional Medicine – Rehabilitation, Phong Dien District Medical Center and Phu Vang District Medical Center in Thua Thien – Hue Province, Thang Binh District Medical Center in Quang Nam Province. After more than a year of operation, the Transition House model has achieved certain effects in helping patients and persons with disabilities get acquainted and adapt before including in the community.

The workshop is the 4th session in a series of activities that both visit reality, study, share about the transfer home as well as meet directly with persons with disabilities who benefit directly from the model. Under the important contributions of the delegates, the model will certainly continue to improve to better, meeting the needs of persons with disabilities as well as health facilities in the future.

Connecting circles - A journey in mental healthcare

On October 11 and 12, 2025, in the communes of Cua Viet, Vinh Linh, and Ai Tu in Quang Tri province, three groups named "Hope," "Faith" and "Sustainability" launched the first group meeting in a series of five psychological support sessions dedicated to persons with disabilities and their family members facing emotional and mental difficulties. This activity marks the start of the psychological support group model under the project "Independent living and Social inclusion for persons with disabilities in Quang Tri province - Vietnam" implemented by the Action to the Community Development Institute (ACDC) and funded by Green Cross Switzerland (GCCH).
